After your inpatient stay in a hospital or nursing facility, you’re eligible to receive 2 meals per day for 14 days—up to 28 nutritious meals delivered to your door at no additional cost to you. (Limited to 4 times per year.)

Humana Care Management

Humana Care Management is a service offered through your Humana plan at no extra cost for Humana Medicare Advantage members with a qualifying condition. Our care managers will talk with you and help you make the best choices regarding your health. Eligible members will develop a care plan with a care manager, who will check in on your progress. We will then follow up to help make sure the service is working for you.

Power of Attorney

You may submit your Power of Attorney (POA) documents to Humana by:

  1. Completing a POA form online
  2. Mailing or faxing
    Be sure to include additional POA documentation detailing the:
  • Member name and date of birth,
  • Humana member ID number (it begins with an “H” and can be found on the ID card), and
  • Name of the person or organization appointed as POA; it should match the information in the legal document.
