How to search for a dentist:

  • Select “Provider search”
  • Enter your ZIP code
  • Select a lookup method; either “DHMO” or “PPO,” then click “Select”
  • For DHMO, choose “HS195 DHMO/Prepaid Network” as your network
  • For PPO, choose “PPO/Traditional Preferred”  as your network
  • Choose your search category and dentist’s name or specialty
  • Select “search”
