  • Humana Community Navigator tool

    Humana provides assistance for MSU members through the Community Navigator tool. Find food assistance, loneliness resources and other free or reduced cost programs, including new programs for the COVID-19 pandemic. Once you select the link, please enter your home’s ZIP code to find programs near you.

  • Humana Provider Finder

    Physician Finder Plus® is your online healthcare provider look-up tool. You’ll find nearby doctors, hospitals, pharmacies and other healthcare providers in the Humana network. You can get phone numbers, addresses and maps, customize your search by specialty and see the distance from your home or work. Once you select the link below, please choose “Insurance through your Employer” and select “Humana/Choice Care Network” as your network in the “Provider Finder” section. Also, please ensure you select the correct dropdown when selecting your provider type.
