Beginning with the 2022 tax year (Jan. 1, 2023), only Members living in states with laws that require reporting of health coverage will continue to automatically receive a paper copy of the Form 1095-B for state filing tax purposes. All other members wanting a paper copy will need to request one by visiting MyHumana or calling 800-4HUMANA (800-448-6262).
An electronic copy of form 1095-B will be posted to myhumana.com for all members whom Humana files a 1095-B with the IRS and states. All members will have the ability to download and print the form themselves.
Form 1095-B will be made available to you no later than the annual deadline set by the IRS. Please visit Humana’s website and click the “Through your employer” tab for additional information.
Humana’s Medical healthcare plans offer a variety of benefit designs and include prescription drug coverage and offer telemedicine. You can select a plan based on cost, provider network, or your medical needs based on your location.

The Humana Medicare Advantage preferred provider organization (PPO) plan is offered to eligible members of the Federal Employees Health Benefits (FEHB) Program. It’s a nationwide plan that takes a total approach to your health by covering your doctors, hospitalization and prescription drugs in 1 simple plan.
For federal members including TRICARE® retirees, Humana offers two great plans to help you achieve better oral health.
Both plans offer an unlimited annual maximum benefit, implant coverage, Adult and Child orthodontia, no waiting periods and no referrals required.

High PPO annual maximum will be changed from $5,000 to unlimited.

The Virtual benefits fair is an online “health fair” that houses plan information in a single online location. This enables Federal employees to browse exhibits, view content and connect with program representatives via an online chat. The Virtual Benefits Fair is available 24/7 during the entire Open Season.

To access Humana’s Special Discounts Program, sign in to MyHumana.com, go to the “Menu” tab at the top and scroll down to “Coverage” and then scroll down to “Special Discounts.”
You may submit your Health Risk Assessment (HRA) using the digital form (in English) or print the form in Spanish and fax to 855-408-7841.
The form should be completed and signed in all parts.
Please visit Quest Diagnostics to complete your Health Risk Assessment and schedule a wellness screening.
You will be asked to enter the registration key “FEHBP23” during the registration process.

You’ll answer some basic questions to get a confidential health profile tailored just for you. Once you complete your Health Assessment and your wellness screening, a personal report comes with recommendations for improving your health that you can share with your doctor.
The knowledge you’ll gain from the report may help you discover some additional ways to get or stay healthy.
A voluntary biometric screening helps give you a true picture of your health. You’ll not only know your numbers, but you’ll be able to understand them, so you can take charge of your health. It may help you feel more empowered to make healthier choices so you can be your best self.
The voluntary Biometric Screening measures a range of health indicators, such as waist circumference, body mass index (BMI), blood pressure, cholesterol, triglycerides and blood sugar. You will receive a report of your results and can learn about possible risk factors you have.
Note: You will be required to read and acknowledge a Notice Regarding Wellness Program prior to completing the Biometric Screening.
While fasting is not required, it’s often suggested that you not eat or drink (aside from water) for 9 to 12 hours before your test to get the most accurate results. Talk with your doctor if you are uncertain whether you should fast or not.

Quitting tobacco is not easy, but you don't have to do it alone. Your Federal Employee Health Benefits can help. You have access to proven cessation treatments that can ease withdrawal symptoms and help you succeed in quitting.
The benefit covers treatment for all forms of tobacco use, including cigarettes, snuff and chewing tobacco. It includes 2 quit attempts per year with up to 4 tobacco cessation discussion sessions of at least 30 minutes each, including proactive telephone discussions, group discussions and individual discussions.

This is a summary of the features of the Humana HDHP, HMO, CoverageFirst and Value Plan. Humana is a Medicare Advantage HMO, PPO and PFFS organization and a stand-alone prescription drug plan with a Medicare contract. Humana is also a Coordinated Care plan with a Medicare contract and a contract with the state Medicaid program. Enrollment in any Humana plan depends on contract renewal.
To take full advantage of the savings available with your benefits, choose a doctor within your network for routine care. With the exception of emergencies and urgent care situations, it may cost more for covered services received outside the network.
Out-of-network/non-contracted providers are under no obligation to treat Humana plan members, except in emergency situations. Please call our customer service number or see your brochure for more information, including the cost-sharing that applies to out-of-network services.
