
1095-B reporting
Beginning with the 2022 tax year (Jan. 1, 2023), only Members living in states with laws that require reporting of health coverage will continue to automatically receive a paper copy of the Form 1095-B for state filing tax purposes. All other members wanting a paper copy will need to request one by visiting MyHumana or calling 800-4HUMANA (800-448-6262).
An electronic copy of form 1095-B will be posted to myhumana.com for all members whom Humana files a 1095-B with the IRS and states. All members will have the ability to download and print the form themselves.
Form 1095-B will be made available to you no later than the annual deadline set by the IRS. Please visit Humana’s website and click the “Through your employer” tab for additional information.

Humana Medicare Advantage PPO: What is it?
The Humana Medicare Advantage preferred provider organization (PPO) plan is offered to eligible members of the Federal Employees Health Benefits (FEHB) Program. It’s a nationwide plan that takes a total approach to your health by covering your doctors, hospitalization and prescription drugs in 1 simple plan.
Humana Dental Plan with High PPO and Standard Advantage EPO Options
For federal members including TRICARE® retirees, Humana offers two great plans to help you achieve better oral health.
Both plans offer an unlimited annual maximum benefit, implant coverage, Adult and Child orthodontia, no waiting periods and no referrals required.
Dental High PPO Plan highlights:

You may submit your Health Risk Assessment (HRA) using the digital form (in English) or print the form in Spanish and fax to 855-408-7841.
The form should be completed and signed in all parts.

You will be asked to enter the registration key “FEHBP23” during the registration process.

How the assessment works
You’ll answer some basic questions to get a confidential health profile tailored just for you. Once you complete your Health Assessment and your wellness screening, a personal report comes with recommendations for improving your health that you can share with your doctor.
The knowledge you’ll gain from the report may help you discover some additional ways to get or stay healthy.
How the wellness screening works
A voluntary biometric screening helps give you a true picture of your health. You’ll not only know your numbers, but you’ll be able to understand them, so you can take charge of your health. It may help you feel more empowered to make healthier choices so you can be your best self.
The voluntary Biometric Screening measures a range of health indicators, such as waist circumference, body mass index (BMI), blood pressure, cholesterol, triglycerides and blood sugar. You will receive a report of your results and can learn about possible risk factors you have.
Note: You will be required to read and acknowledge a Notice Regarding Wellness Program prior to completing the Biometric Screening.
Before your visit
Limit your food intake
While fasting is not required, it’s often suggested that you not eat or drink (aside from water) for 9 to 12 hours before your test to get the most accurate results. Talk with your doctor if you are uncertain whether you should fast or not.

HumanaBeginnings® is a program that advises members on how best to care for themselves during their pregnancy. Experienced, registered nurses provide education, support and guidance throughout pregnancy.

Tobacco cessation
Quitting tobacco is not easy, but you don't have to do it alone. Your Federal Employee Health Benefits can help. You have access to proven cessation treatments that can ease withdrawal symptoms and help you succeed in quitting.
The benefit covers treatment for all forms of tobacco use, including cigarettes, snuff and chewing tobacco. It includes 2 quit attempts per year with up to 4 tobacco cessation discussion sessions of at least 30 minutes each, including proactive telephone discussions, group discussions and individual discussions.
Case and care management
Humana offers a variety of programs for patients who need care for medical situations, support for chronic conditions, maternity and transplant services. Through these programs, care managers collaborate with physicians and other healthcare professionals to help patients manage their healthcare needs while addressing their physical, behavioral, cognitive, social and financial needs.
Telemedicine (also known as telehealth or virtual care) uses information technology and telecommunications to provide virtual clinical care to patients. Patients can interact with providers through video and app technology by using smartphones, tablets and laptops.
With Humana’s telemedicine benefit delivered by Doctor On Demand, you can:
- Connect with a physician from one of Doctor On Demand’s U.S. board-certified doctors. For Puerto Rico members, connect with a physician of MDLive’s U.S. board-certified doctors with a state license.
- See a doctor 24 hours a day, 7 days a week from any location.
- Only pay your Primary Care Physician co-payment (for Puerto Rico members, only pay your non-directed primary care physician copayment).
Your primary care physician can access your telemedicine visit at your request. If medically necessary, the telemedicine doctor can send a prescription to a preferred pharmacy.
