Members living in states with laws that require reporting of health coverage will continue to automatically receive a paper copy of the Form 1095-B for state filing tax purposes. All other members wanting a paper copy will need to request one by visiting MyHumana or calling 800-4HUMANA (800-448-6262).
An electronic copy of form 1095-B will be posted to myhumana.com for all members whom Humana files a 1095-B with the IRS and states. All members will have the ability to download and print the form themselves.
Form 1095-B will be made available to you no later than the annual deadline set by the IRS.

Our network includes dentists and specialists near you, with office hours that work for you. Our Find a Dentist tool will show you dental providers covered by your plan.
To take full advantage of the savings available with your benefits, choose a doctor within your network for routine care. With the exception of emergencies and urgent care situations, it may cost more for covered services received outside the network.
Out-of-network/non-contracted providers are under no obligation to treat Humana plan members, except in emergency situations. Please call our customer service number or see your brochure for more information, including the cost-sharing that applies to out-of-network services.
